About the Job:
About Us: A Hug Away Healthcare is a leading provider of home health and hospice services. We are seeking a compassionate and experienced Home Health and Hospice Nurse to join our team. This role involves providing high-quality care to patients in their homes and managing their cases effectively using the WellSky EMR system.
Key Responsibilities:
- Provide comprehensive nursing care to home health and hospice patients, including assessments, care planning, and implementation of care plans.
- Utilize the WellSky EMR system to document patient care accurately and efficiently.
- Perform case management duties, including coordinating care with other healthcare providers, managing patient records, and ensuring continuity of care.
- Educate patients and their families about their conditions, treatment plans, and available resources.
- Monitor and evaluate patient progress, adjusting care plans as necessary.
- Administer medications and treatments as prescribed by physicians.
- Conduct regular home visits to assess patient needs and provide necessary care.
- Communicate effectively with patients, families, and the healthcare team to ensure optimal patient outcomes.
- Participate in interdisciplinary team meetings to discuss patient care and develop comprehensive care plans.
- Ensure compliance with all regulatory requirements and company policies.
Qualifications:
- Valid Registered Nurse (RN) license in the state of Texas.
- At least 1 year of experience in home health and hospice nursing.
- Proficiency in using the WellSky EMR system.
- Strong case management skills and experience.
- Excellent organizational and time-management skills.
- Compassionate and patient-centered approach to care.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver’s license and reliable transportation.
Preferred Qualifications:
- Certification in hospice and palliative care.
- Experience in the Kingwood, TX area.
- Familiarity with Medicare and Medicaid regulations.
- Experience working with children is highly preferred but not required.
