About the Job:

About Us:
A Hug Away Healthcare is a leading provider of home health and hospice services. We are seeking a compassionate and experienced Home Health and Hospice Nurse to join our team. This role involves providing high-quality care to patients in their homes and managing their cases effectively using the Kinnser EMR system.

Key Responsibilities:

Provide comprehensive nursing care to home health and hospice patients, including assessments, care planning, and implementation of care plans.
Utilize the Kinnser EMR system to document patient care accurately and efficiently.
Perform case management duties, including coordinating care with other healthcare providers, managing patient records, and ensuring continuity of care.
Educate patients and their families about their conditions, treatment plans, and available resources.
Monitor and evaluate patient progress, adjusting care plans as necessary.
Administer medications and treatments as prescribed by physicians.
Conduct regular home visits to assess patient needs and provide necessary care.
Communicate effectively with patients, families, and the healthcare team to ensure optimal patient outcomes.
Participate in interdisciplinary team meetings to discuss patient care and develop comprehensive care plans.
Ensure compliance with all regulatory requirements and company policies.

Qualifications:

Valid Registered Nurse (RN) license in the state of Texas.
At least 1 year of experience in home health and hospice nursing.
Proficiency in using the Kinnser EMR system.
Strong case management skills and experience.
Excellent organizational and time-management skills.
Compassionate and patient-centered approach to care.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Valid driver’s license and reliable transportation.

Preferred Qualifications:

Certification in hospice and palliative care.
Experience in the Kingwood, TX area.
Familiarity with Medicare and Medicaid regulations.
Experience working with children is highly preferred but not required.
