Job Details:

Currently licensed with the state of Texas.
Excellent verbal and written communication skills.
Experience with electronic medical records is strongly desired.
Home Health or hospice assessments including consents in the home, hospital or facility.
1 year of Hospice and Home health experience highly recommended.
Kinnser experience preferred.
Be available by phone during normal working hours, when on call, and when on back up call.
Performs patient visits according to the Plan of Care or as patient need dictates.
Deliver care and instruction that is appropriate to the patient’s plan of care.
Coordination of care with wound care agencies, doctors and team members via Kinnser or IDG.
Perform case management activities for assigned patients.
Respond to verbal and written issues, concerns, or complaints from home care patients, families, physicians, nurse.
Comprehensive Assessments of patients utilizing the OASIS data sets or Hospice HIS data set.
May entail GIP care.
Meet with the IDT/case conference team once every other week.
QA for clinicians on the care team.
Experience working with children highly considered.
Coordinate schedule to make most use of time during the day.
May require driving outside of the residential area.
