Job Details:
- Currently licensed with the state of Texas.
- Excellent verbal and written communication skills.
- Experience with electronic medical records is strongly desired.
- Home Health or hospice assessments including consents in the home, hospital or facility and
- 1 year of Hospice and Home health experience highly recommended.
- Kinnser experience preferred
- Be available by phone during normal working hours, when on call, and when on back up call.
- Performs patient visits according to the Plan of Care or as patient need dictates.
- Deliver care and instruction that is appropriate to the patient’s plan of care
- Coordination of care with wound care agencies, doctors and team members via Kinnser or IDG.
- Perform case management activities for assigned patients.
- Respond to verbal and written issues, concerns, or complaints from home care patients, families, physicians, nurse.
- Comprehensive Assessments of patients utilizing the OASIS data sets or Hospice HIS data set
- May entail GIP care
- Meet with the IDT/case conference team once every other week
- QA for clinicians on the care team
- Experience working with children highly considered
- Coordinate schedule to make most use of time during the day
- May require driving outside of the residential area

Experience:
- Home health and/or hospice 1 year Required
- Nursing: 1 year (Preferred)

- BLS Certification (Preferred).
- RN Texas or Compact state
