About the Job
We are seeking a compassionate, confident, and highly organized Certified Nursing Assistant (CNA) to join our team as a Hospital Liaison & Intake Specialist. This role is ideal for a CNA who enjoys working directly with patients, communicating with hospital staff, and supporting families through important care decisions. In this position, you will represent our agency within local hospitals, assist with obtaining consents, support insurance verification, and participate in intake and light marketing activities. A company vehicle is provided for all work-related travel, including hospital visits and field responsibilities. 

Key Responsibilities:
  • Visit patients in hospitals to explain services and obtain required consent forms.
  • Assist patients and families with insurance verification and authorization calls.
  • Communicate professionally with nurses, case managers, and hospital personnel. 
  • Complete intake procedures accurately and ensure all documentation is gathered. 
  • Represent the agency with professionalism and empathy at all times. 
  • Use the company vehicle to travel to hospitals and patient locations as needed. 
  • Maintain clear communication with the office regarding patient updates and completed tasks. 
  • Build positive relationships with hospital staff and support basic marketing/outreach efforts. 

Required Qualifications:
  • Active CNA license in good standing.
  • Strong communication and interpersonal skills. 
  • Comfortable speaking with patients, families, and hospital staff. 
  • Experience with intake processes or insurance verification is a plus. 
  • Ability to work independently and stay organized. 
  • Professional appearance and demeanor. 
  • Reliable work ethic and strong attention to detail. 
  • Valid driver’s license (company vehicle provided for work duties).
