Job Summary:
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our home health and hospice agency as a Case Manager. This office-based position involves coordinating patient care, ensuring quality assurance (QA) standards, and providing administrative support. The ideal candidate will have experience in hospice care as well as some home health, strong organizational skills, and a commitment to delivering exceptional patient care.
Key Responsibilities:
Quality Assurance (QA):
- Ensure compliance with all regulatory and accreditation standards for both state and federal.
- Conduct regular audits of patient charts and documentation.
- Identify areas for improvement and implement corrective actions.
- Participate in QA meetings and contribute to the development of policies and procedures.
Administrative Support:
- Maintain accurate and up-to-date patient records.
- Assist with the onboarding and training of new staff.
- Handle patient inquiries and provide information about services.
- Collaborate with the billing department to ensure accurate and timely billing of services.
- QA medical records and ensure clinicians are documenting according to federal guidelines.
- Assist in preparing documents for payor audits.
- Coordinate and Attend IDT meetings.
- Act as advisory for owners and administrators.
Communication:
- Serve as a liaison between patients, families, and healthcare providers.
- Provide education and support to patients and their families regarding care plans and treatment options.
- Communicate effectively with all members of the healthcare team to ensure coordinated and efficient patient care.
Qualifications:
Education and Experience:
- Current and valid LVN license in the state of employment.
- Minimum of 2 years of experience in hospice and home health preferred.
- Experience with quality assurance processes and regulatory compliance.
Skills and Abilities:
- Strong organizational and time-management skills.
- Excellent communication and interpersonal skills.
- Proficient in electronic medical records (EMR) systems and Microsoft Office Suite.
- Ability to work independently and as part of a team.
- Compassionate and patient-centered approach to care.
Additional Requirements:
- Ability to work in an office setting with standard office equipment.
- Occasional travel to patient homes and healthcare facilities as needed.
