Key Responsibilities:
Leadership and Team Management:
- Lead, mentor, and manage the nursing team, fostering a positive and inclusive work environment.
- Develop and implement nursing policies and procedures to ensure the highest standards of patient care.
- Promote teamwork and collaboration across the organization.
- Act as a key driver for the growth of the agency by building strong relationships within the clinical community and serving as a clinical resource in marketing efforts.
- Assist with hiring talented staff, ensuring alignment with the company's vision and goals.
- Ensure staff education and ongoing development, fostering a culture of continuous learning and professional growth.
- Collaborate with management to develop processes that enhance efficiency and organization.
Patient Care and Clinical Excellence:
- Oversee patient care plans, ensuring they are tailored to meet the needs of each individual.
- Conduct field visits to assess patient conditions and provide direct care as needed.
- Work closely with physicians, therapists, and healthcare professionals to ensure a holistic approach to care.
- Drive innovation in patient care processes, ensuring that both adult and pediatric patients receive the best possible care.
Documentation and Compliance:
- Ensure accurate and timely documentation of patient care in compliance with regulatory and accreditation standards.
- Conduct regular audits of patient charts and records to maintain quality and accuracy.
- Stay informed of state and federal regulations, ensuring compliance in all areas of operation.
Quality Assurance and Improvement:
- Lead quality assurance initiatives aimed at improving patient outcomes.
- Participate in QA meetings, contributing to the development of policies and procedures.
- Identify areas for improvement and collaborate on action plans to address them.
Education and Training:
- Provide education and training to nursing staff on best practices, policies, and procedures.
- Mentor and support new staff, ensuring a smooth onboarding experience.
- Promote continuous learning and professional development among the nursing team.
Community Engagement and Cultural Competence:
- Play a key role in positioning the agency as a community leader in hospice and home health services.
- Work effectively with diverse age groups, cultures, and ethnicities, fostering a respectful and inclusive environment.
- Serve as a representative of the agency in the community, participating in creative and educational outreach projects.
- Help lead remote teams from diverse backgrounds and geographies.
Qualifications:
**Education and Experience:**
- Current and valid RN or LVN license in the state of Texas.
- Minimum of 3 years of nursing experience, with at least 1 year in a leadership role.
- Experience in home health and hospice care (or a strong ability to learn either- quickly).
- Experience working with or willingness to work with pediatric patients.
- Bilingual (Spanish preferred; stipend offered).
Skills and Abilities:
- Strong leadership, management, and mentorship skills.
- Excellent organizational and time-management abilities.
- Knowledge of Medicare guidelines and compliance requirements.
- Ability to foster relationships with families and provide the best possible care for all ages.
- Proficiency in Kinnser (or willingness to learn).
- Ability to balance multiple responsibilities while maintaining excellent patient care standards.
